Please fill in the details on this form if you have been told to do so by a staff member and you are a new student booking or a returning student booking.
By choosing "Submit" you are agreeing to the Following Holiday Lesson Terms and Conditions of Paragon Swim Centre Pty Ltd.   

1.  A $25 Deposit is required to confirm Holiday Lesson Booking.

2.  All lessons must be paid for on or before the first lesson.

3.  Deposits are NON REFUNDABLE after the first lesson.

4.  Payments can be made online into the Paragon Swim Centre P/L Bank Account - BSB 105135  Acct: 6048 9040  Acct Name: Paragon Swim Centre  Reference: Students Last Name and First Name initial.

5.  Please note that if lessons are missed during the holiday lessons     NO MAKE UP LESSONS WILL BE ARRANGED.

6.  Children are NOT ALLOWED INTO THE WATER BEFORE THEIR LESSON and NOT ALLOWED TO STAY IN THE WATER AFTER THEIR LESSON IS FINISHED.

7.  No Make ups from Term Lessons can be used in Holiday Lessons.

8.  There are no family discounts available.

9.  You are booking a position and are required to attend every session.
